* Program Notice *

◆ You may find that the Zipcode lookup feature claims that it can't find your address. This may be due to a new limitation imposed by USPS on how frequently we are allowed to make lookup requests. If this happens to you, try waiting for 2 hours and then submit this form again.

* Communication Notice *

◆ We use email as our primary form of communication.
◆ After you have submitted the form, you will receive a confirmation email within 5 minutes. If not, check that the address you entered was correct, or if our message has gone to your spam folder, or that you haven't used up your storage space.
◆ After receiving the confirmation, it may take up to two weeks to schedule your pickup time, so please be patient.

* Eligibility Requirements for Receiving Toys *

◆ We serve children from infants up to 13 years old.
◆ You must live in the City of Portland (Zip codes are checked)
◆ Your income should be at or below the Federal Poverty Guidelines (list)
◆ Grandparents cannot request toys for their grandchildren unless they have legal custody.

* Toy Pickup Appointments *

◆ If approved, we will send you an email with an assigned pickup day. Due to storage space limitations, we cannot hold your toy box past that pickup day.
◆ When you come, bring photo identification for yourself and a piece of mail that shows your current address, and bring identification for each child that shows his/her date of birth. (Examples)
◆ Grandparents claiming legal custody of grandchildren must bring confirming documentation.
